I use it for feedback from my ASPSmith training classes. A search for "survey" on ASPToday should find it no problem, although you'll probably then have to subscribe to get to the code. It's very nice - it lets you set up multiple surveys with multiple sections and a variety of question and answer formats, and it provides reports of the results as well.
